South West AHSN: Supporting digital-first pathways Programme

Optimising digital interactions to improve patient outcomes

THE RESPONSE TO COVID-19

To reduce the risk of COVID-19 infection, health and care systems have rapidly implemented digital technology to reduce the need for physical contact between patients and clinicians. These ‘digital-first’ care pathways are enabling health and care teams to rapidly exchange information and clinically manage patients at a time of social distancing.

In the South West, the availability of digital tools to support care pathways has grown significantly over the last few months. There is still potential to optimise how they are used; improving access to digital care for patients, enhancing the patient experience, and creating more effective interactions between patients and clinicians.

We are supporting our health and care partners across Somerset, Devon and Cornwall & the Isles of Scilly to facilitate effective and equitable digital-first care. Together we are aiming to identify areas of best digital practice that can be rapidly shared and spread across the region to improve patient outcomes.
